• Standard, Full-Review IRB Application
  For all general research projects involving human subjects, fill out this form COMPLETELY, and do not forget to include your and your faculty sponsor’s signatures.
• Exempt Research Application
  This form is for applications which meet the criteria for exemption from full IRB review. To see if your project is exempt from review, open this document and complete the checklist.
• Consent Form Template
  Highly Recommended! Use this template as a guide to develope a consent form for your own project. We highly encourage you to use this form- the vast majority of applications are rejected due to poorly constructed consent documents.

• Principal/Administrator’s Consent
  This form is used to obtain a principal or administrator’s consent to perform research at their institution (outside of Truman State University). The principal or administrator must be given a copy of the research proposal along with any consent documents to be used. This form must be signed and submitted to the Grants Office before approval of projects requiring it.
