Highly Recommended! Use this template as a guide to develop a consent form for your own project. We highly encourage you to use this form- the vast majority of applications are rejected due to poorly constructed consent documents.

This form is used to obtain a principal or administrator's consent to perform research at their institution (outside of Truman State University). The principal or administrator must be given a copy of the research proposal along with any consent documents to be used. This form must be signed and submitted to the Grants Office before approval of projects requiring it.
